The Lifetime LASIK Program is our commitment to exceptional care for our patients throughout their lifetime. Enrollment is automatic when your initial laser vision correction is completed at Indiana LASIK Centers. It’s that easy! Your optometrist will maintain the appropriate records to determine eligibility.
You will receive your Lifetime LASIK Program passport from Indiana LASIK Centers the day of your procedure. The staff will explain LASIK enhancement eligibility and review the details of the program. The program booklet allows you to easily track your follow-up care and provides you the reassurance of a lifetime of great vision. All eligible enhancements will be performed at the much reduced cost of $399 per eye.
Eligibility for Enhancement
- Initial LASIK treatment, for the correction of myopia, hyperopia and/or astigmatism, was performed at ILC
- Prior to your procedure, if your myopic correction is between -0.50 and -12.00 or +0.50 to +5.00 with no greater than 4.00 diopters of astigmatism, and your best corrected acuity was 20/40 or better
- You have completed the course of post-treatment care, including all follow-up visits, as prescribed by your optometrist
- Any refractive condition of 0.75 diopters greater than your targeted outcome that exists or develops after the procedure may be retreated, provided your doctor agrees that an enhancement is medically safe, appropriate, and has a high probability of correcting your vision to at least 20/40.
